Proof of Identity (any one)
- Aadhaar Card — the most commonly accepted and recommended
- PAN Card
- Voter ID (EPIC)
- Driving Licence
- Government / PSU employee identity card
Proof of Address (any one)
- Aadhaar Card — accepted as both identity and address proof
- Utility bills — electricity, water, gas, telephone (not older than 3 months)
- Bank account statement / passbook (not older than 3 months)
- Rental agreement (notarised)
- Income Tax Assessment Order
- Voter ID / Election Commission certificate
- Driving licence
💡 Aadhaar as dual proof: Aadhaar Card is the simplest option as it serves as both identity proof AND address proof in a single document. Ensure your Aadhaar address matches the address you provide in the passport application form.
Proof of Date of Birth (any one)
- Birth certificate issued by Municipal Corporation / Gram Panchayat
- Transfer certificate from school (with date of birth)
- Matriculation (Class 10) certificate
- Policy bond issued by a life insurance company
- Aadhaar Card (for applicants born on or after 26 January 1989)
Passport Photographs
Photographs must comply with ICAO biometric standards:
- Size: 51mm × 51mm
- Face occupying 25–35mm of the image height (chin to top of head)
- Plain white background — no patterns or gradients
- Eyes open, looking directly at camera, neutral expression
- No glasses (MEA strongly discourages, may cause rejection)
- No head coverings except for religious reasons
- Taken within the last 6 months
- High resolution, sharp focus, no shadows on face or background
For Fresh Passport (First-time Applicant)
- Proof of identity (any one from list above)
- Proof of address (any one from list above)
- Proof of date of birth (any one from list above)
- Passport-size photographs (2–4 as required)
- Online application form (filled and submitted on Passport Seva portal)
- Appointment confirmation printout
For Passport Renewal (Re-issuance)
- All fresh passport documents listed above
- Original existing passport + self-attested photocopy of all relevant pages (data page, address page, last and penultimate pages)
- Address proof if address has changed from existing passport
For Tatkal Passport
- All standard documents (identity, address, DOB proof, photographs)
- Existing passport (for re-issuance)
- Annexure F — self-declaration form (download latest version from passportindia.gov.in)
- Verification certificate from gazetted government officer (if applicable)
For Minor Passport (Below 18 Years)
- Birth certificate of the child
- Aadhaar Card of the child (if available)
- Passport-size photographs of the child
- Original passport of both parents + self-attested photocopies
- Aadhaar Card of both parents
- Annexure D (declaration for minors) if one parent is absent
- Annexure C (consent from parent/guardian) — required when child travels alone
- School bonafide / identity card
Self-Attestation Rules
All photocopies must be self-attested — write "Self-Attested" and sign with the same signature as used in the application form. Do not get photocopies notarised unless specifically required.
⚠️ Critical: Ensure the name on all documents matches exactly. Any discrepancy between Aadhaar, existing passport, and birth certificate can cause your application to be rejected or returned.
